station practice.

Create a useful clinical record that distinguishes observations, actions, communication and response. Your notes should help the next clinician continue care.

What to practise

Practise completing a fictional observation record, a fluid balance entry or a progress note. Read existing entries first, use the supplied time and identifiers and make uncertainty visible.

This is a preparation category, not a prediction of a particular examination station. Read the candidate instructions and respond to the person and task you are given.

Make your approach clear

  • Record measured findings with units, time and relevant context.
  • Document whom you contacted, the response and the agreed plan.
  • Follow the relevant correction process rather than hiding or guessing missing information.

Rehearsal prompt: “I will explain what I am checking, what I have found and what needs to happen next.” Adapt the words to the actual findings; do not memorise a claim you cannot support.

Common ways to lose the thread

  • Writing only “doctor aware” without the response or plan.
  • Recording care before it has happened.
  • Leaving essential information only on a personal reading note.
