Your scenario and task

You are the RN in an aged care facility. Lucia Bianchi, aged 87, has declined her due oral medicine because she fears choking. A care worker suggests signing it as given to avoid an omission report. Assess the concern, respond safely and write an accurate progress and medicine record. Complete the supplied practice record. A second RN is available. You have eight minutes; describe and select physical actions in this online exercise.

What this type of station practises

Create a useful clinical record that distinguishes observations, actions, communication and response. Your notes should help the next clinician continue care.

In “Recording a resident’s declined medicine”, use the task in the reading card to decide what to assess, explain and record in aged-care. Practise completing a fictional observation record, a fluid balance entry or a progress note. Read existing entries first, use the supplied time and identifiers and make uncertainty visible.

These are general rehearsal aims, not the answers to this scenario. Respond to information obtained during practice rather than a memorised solution.

Say-and-do preparation

Prepare the sequence and language with a related checklist. Online rehearsal develops thinking and explanations; physical technique needs supervised in-person practice.
