How to use this checklist
Read the fictional task first. Work through each prompt out loud, using only the information supplied. Where a clinical detail is missing, say what you need to clarify rather than inventing it.
Compare the supplied order with the fictional current infusion.
“I will establish what is prescribed and what is currently running.”
Explain the patient, access site and fluid balance assessment needed.
“I will assess the person and the access site as well as reviewing the equipment.”
Describe how you will clarify a discrepancy before changing care.
“This discrepancy needs clarification before I change the infusion.”
Plan documentation and review of the response.
“I will record the findings, the plan and the response to care.”
Check your explanation
- Read the order and establish what is currently running before making changes.
- Explain the access site and patient observations you need to check.
- Describe what you will record and when you will reassess the response.
Watch for these omissions
- Changing an infusion without resolving a mismatch in the order.
- Focusing on the pump while overlooking the patient.
- Documenting intake without considering the rest of the fluid balance.
Connect it to a scenario
Practise reviewing a fictional infusion order, describing a site assessment and handing over a fluid balance concern. Calculation practice should retain units and use the supplied order and equipment details.
Repeat with a different fictional situation to check that your explanation responds to the task rather than a memorised script. Review the current ACSQHC Communicating for Safety Standard and RN Standards for Practice alongside supervised skills training.
