Your scenario and task

You are the RN on a hospital ward. Emily Patel, aged 47, has observations taken at 10:00. The electronic practice record contains an oxygen saturation entry that differs from your measured value. Check the current patient, correct the record transparently and document any escalation. Complete the supplied practice record. You and the patient are in the room; assistance can be called. You have eight minutes; describe and select physical actions in this online exercise.

What this type of station practises

Create a useful clinical record that distinguishes observations, actions, communication and response. Your notes should help the next clinician continue care.

In “Correcting an observation entry safely”, use the task in the reading card to decide what to assess, explain and record in ward. Practise completing a fictional observation record, a fluid balance entry or a progress note. Read existing entries first, use the supplied time and identifiers and make uncertainty visible.

These are general rehearsal aims, not the answers to this scenario. Respond to information obtained during practice rather than a memorised solution.

Say-and-do preparation

Prepare the sequence and language with a related checklist. Online rehearsal develops thinking and explanations; physical technique needs supervised in-person practice.
