Your scenario and task

Eileen Fraser has a prescribed daily fluid limit and a partially completed intake/output record. She says her shoes feel tighter. Reconcile the recorded totals, assess the resident and document a clinically useful review plan. Do not change the fluid limit. The patient answers personal questions; the examiner supplies requested examination findings. Dr Taylor Singh is reachable by phone.

What this type of station practises

Create a useful clinical record that distinguishes observations, actions, communication and response. Your notes should help the next clinician continue care.

In “A fluid balance total that hides a trend”, use the task in the reading card to decide what to assess, explain and record in aged-care. Practise completing a fictional observation record, a fluid balance entry or a progress note. Read existing entries first, use the supplied time and identifiers and make uncertainty visible.

These are general rehearsal aims, not the answers to this scenario. Respond to information obtained during practice rather than a memorised solution.

Say-and-do preparation

Prepare the sequence and language with a related checklist. Online rehearsal develops thinking and explanations; physical technique needs supervised in-person practice.
