Your scenario and task
You are the RN on a hospital ward. Samuel Lee, aged 76, is preparing for discharge after an uncomplicated admission. His signed plan includes a GP appointment in three days and a reconciled unchanged medicine list. Make the discussion accessible, explain the discharge plan and check understanding. Complete the supplied practice record. You and the patient are in the room; assistance can be called. You have eight minutes; describe and select physical actions in this online exercise.
What this type of station practises
Help the person understand, participate and make informed choices. Good communication changes with the person’s needs rather than following a fixed speech.
In “Hearing-accessible discharge teaching”, use the task in the reading card to decide what to assess, explain and record in ward. Practise discharge education, a conversation with an anxious relative or explaining an unfamiliar procedure. Ask about language, access and cultural needs rather than making assumptions.
These are general rehearsal aims, not the answers to this scenario. Respond to information obtained during practice rather than a memorised solution.
Say-and-do preparation
Prepare the sequence and language with a related checklist. Online rehearsal develops thinking and explanations; physical technique needs supervised in-person practice.
