Your scenario and task

Helen Grant is under ED observation after a fall with a head strike. The neurological chart shows a new change. Escalate using ISBAR and record the confirmed response. Do not delay urgent support to complete the form. The patient is a manikin; address the patient and the examiner voices their answers and supplies findings. Dr Taylor Singh is reachable by phone. RN Alex Morgan is available.

What this type of station practises

Make a clear clinical request using Introduction, Situation, Background, Assessment and Recommendation. The listener should understand both the concern and the action needed.

In “Telephone escalation of a falling neurological score”, use the task in the reading card to decide what to assess, explain and record in ed. Practise a telephone escalation, a shift handover or a transfer of care. Prepare a brief opening, relevant background, measured findings and an explicit request. Do not delay emergency assistance to finish a routine call.

These are general rehearsal aims, not the answers to this scenario. Respond to information obtained during practice rather than a memorised solution.

Say-and-do preparation

Prepare the sequence and language with a related checklist. Online rehearsal develops thinking and explanations; physical technique needs supervised in-person practice.
