Your scenario and task
You are the RN on a hospital ward. Yusuf Ali, aged 71, is being treated for a suspected infection and has become more unwell. At 09:00 he was alert, BP 118/70, HR 92, RR 20, SpO2 96% room air and temperature 38.0°C. Assess current urgency, telephone the doctor using ISBAR and document the response. Complete the supplied practice record. Speak to the patient and describe each assessment as you perform it. A second RN is available. The doctor can be telephoned. You have eight minutes; describe and select physical actions in this online exercise.
What this type of station practises
Make a clear clinical request using Introduction, Situation, Background, Assessment and Recommendation. The listener should understand both the concern and the action needed.
In “Telephone escalation for worsening infection”, use the task in the reading card to decide what to assess, explain and record in ward. Practise a telephone escalation, a shift handover or a transfer of care. Prepare a brief opening, relevant background, measured findings and an explicit request. Do not delay emergency assistance to finish a routine call.
These are general rehearsal aims, not the answers to this scenario. Respond to information obtained during practice rather than a memorised solution.
Say-and-do preparation
Prepare the sequence and language with a related checklist. Online rehearsal develops thinking and explanations; physical technique needs supervised in-person practice.
