Your scenario and task

Esther Mensah needs assistance to reposition after a recent illness. A carer has noticed a change over her sacrum. Assess pressure injury risks and the skin change, agree prevention and review, and complete the pressure-risk record. The patient answers personal questions; the examiner supplies requested examination findings. RN Alex Morgan is available.

What this type of station practises

Explore a person’s specific risks and agree a practical prevention plan. Completing a form is one part of an assessment, not the whole task.

In “Pressure damage assessment in darker skin”, use the task in the reading card to decide what to assess, explain and record in aged-care. Use invented histories to discuss falls, skin integrity or a change in mental wellbeing. Consider mobility, cognition, environment, support and what matters to the person.

These are general rehearsal aims, not the answers to this scenario. Respond to information obtained during practice rather than a memorised solution.

Say-and-do preparation

Prepare the sequence and language with a related checklist. Online rehearsal develops thinking and explanations; physical technique needs supervised in-person practice.
