Your scenario and task

You are the RN in an aged care facility. Gordon McLeod, aged 86, has been unusually sleepy since breakfast. His usual cognition is clear, according to the care record. Assess observations and acute change, then explain and arrange the next care step. Complete the supplied practice record. Speak to the patient and describe each assessment as you perform it. A second RN is available. You have eight minutes; describe and select physical actions in this online exercise.

What this type of station practises

Collect a coherent set of observations, notice a change from baseline and communicate your concern. A number becomes useful when you connect it to the person in front of you.

In “New confusion during residential care observations”, use the task in the reading card to decide what to assess, explain and record in aged-care. Rehearse a ward review after a procedure, a change during a home visit or new symptoms in residential care. Decide what to assess first, what to repeat and how to record the time and response.

These are general rehearsal aims, not the answers to this scenario. Respond to information obtained during practice rather than a memorised solution.

Say-and-do preparation

Prepare the sequence and language with a related checklist. Online rehearsal develops thinking and explanations; physical technique needs supervised in-person practice.
